This page describes subsequent delivery reports for the citizen Bruno Test Elmer. For each delivery report the story is told (in Danish), and a reporting example is given.
Bruno med CPR: 1502779995, har indberetter Holbæk kommune (SOR: 451000016003). Bruno har både diabetes type 2 og KOL.
Bruno er af egen læge henvist til afklarende samtale ved Holbæk kommunes forebyggelsestilbud til borgere med kronisk sygdom pga. sin type-2 diabetes. Lægen vurderer at han har brug for fokus på ernæring. Henvisningen modtages af Holbæk kommune d. 2. juni 2022, og samme dag planlægges Brunos afklarende samtale.

Til Brunos afklarende samtale fortæller han både om sin KOL og diabetes, og de mange udfordringer det giver i hverdagen. I samråd med den sundhedsprofessionelle bliver de enige om at fokusere på de problemer der er ift. tilstanden ‘ernæring’ og igangsætte indsatsen ‘madlavning i praksis’
A delivery report is generated containing the Citizen and an Organization (the one delivering the intervention). These are not updated from last time, but they are reported because the new content references these instances.
The delivery report also contains the updated PlannedIntervention (Afklarende samtale).
The new content in the delivery report is an Enconter (Afklarende samtale), a Condition (ernæring), a CarePlan (§119 forbyggelsesforløb) and a PlannedIntervetion (Madlavning i praksis).

Bruno har første gang på holdet ‘Madlavning i praksis’ d. 15. juni, og møder i alt tre gange af to timer. Dette medfører en indberetning til Gateway af tid forbrugt på kontakterne.
De tre rapporter vil være næsten ens, og derfor er det udelukkende rapporten fra 29. juni, der vises her. Læg mærke til at rapporten ikke indeholder tidligere indberettede kontakter - det er fordi de hverken ændrer sig eller refereres.

Bruno bliver indlagt d. 1.-3. juli pga. en forværring i sin KOL. D. 4. juli sender Sygehuslægen en henvisning til kommunen angående iværksættelse af et 119 forløb, gerne med fokus på sygdomsmestring. Fordi Bruno allerede er i §119 forløb planlægges der en behovssamtale (ikke en afklarende samtale).
(d. 6. juli er der igen en kontakt på ‘Madlavning i praksis’. Rapporten dog ikke er medtaget her, fordi den ligner foregående)

Bruno er til behovssamtale. Bruno og den sundhedsprofessionelle bliver enige om, at Bruno bør styrke sin tilstand ‘Sundhedskompetence’ ved at indgå i et hold med fokus på KOL dvs. indsatsen ‘Sygdomshåndtering’. Denne indsats skal køre parallelt med at Bruno gør sit forløb vedr. ‘Madlavning i praksis’ færdig, som han er blevet meget glad for.
(herefter rapporteres en række kontakter på de to forløb, og Madlavning i praksis og ernærings-tilstanden afsluttes. Dette er ikke er med i eksemplet)

Ved sin afslutningssamtale får Bruno afsluttet sin tilstand ‘Sundhedskompetence’ og indsats ‘Sygdomshåndtering’, som er gået rigtig godt. Holbæk kommune har fokus på systematisk opfølgning. Det betyder, at der oprettes et opfølgningsforløb til Bruno i forbindelse med afslutningen.
